Colonial Rehabilitation & Nursing Center in Weymouth is a 174-bed skilled center offering subacute, short-term, long-term and a specialized Memory Care Program for individuals with Alzheimer’s disease or a related memory disorder. Colonial is the first Welch Healthcare rehabilitation & nursing center to provide sub-acute care. Our experience in caring for those who need sophisticated medical, rehabilitative or skilled nursing care make us a logical choice for individuals with more complicated diagnoses.

The Welch Family believes that the compassion and support needed to promote well-being are just as important as the skilled nursing care necessary for a full recovery from an illness or injury. To ensure residents feel at home, Colonial offers private and semiprivate rooms and amenities such as a library, pub, private sitting and living rooms, musical entertainment and a range of activities. Each resident has immediate access to cable TV and private telephone service. Residents enjoy meals in our elegant dining room or in their own room, depending upon individual preference.

Conveniently located a short distance from Routes 3 and 53, the center is easy to access for friends and family. Colonial is accredited by The Joint Commission and recently achieved a 100%, Deficiency-Free Survey by the Massachusetts Department of Public Health in 2008.The center is Medicaid/Medicare Certified and accepts coverage from many private insurers.

Social services, counseling and discharge planning are important elements of the care and services Colonial Rehabilitation & Nursing Center provides. Social workers help to identify a residents psycho-social, mental and emotional needs and then develop individualized plans to meet those needs. Discharge planners define additional services and follow-up resources necessary to ensure a smooth transition from the nursing center to home.
