RN Resident Care Coordinator 32 hours In this position you will oversee private care services, conduct resident assessments, develop care plans, conduct introductory / supervisory visits with home health aides, and serve as a liaison to families and community health care providers. Must be a “Can Do” team player; able to generate excitement and enthusiasm around new service offerings.
