Dear Families, Friends and Guardians:

We have been providing monthly Alzheimer’s - Dementia support group at the Colonial Rehabilitation & Nursing Center for many years. So that we may better better serve your needs at these meetings, we are asking you to share your valuable input by completing this short survey.

Thank you for helping us in our ongoing efforts to improve our services to our residents, their families, as well as caregivers throughout our community.

1.The support group meets from 7PM – 8PM on last Wednesdays of each month.
