ADULT DAY HEALTH CENTERS
Welch Healthcare's Day Health Centers provide a range of
health and social services in a congregate setting,
enhancing the daily lives of the program's participants and
enabling their continued involvement in the community. A Day
Health Center is coordinated with and relates to other
agencies and services, such as in-home services, nursing
homes and hospital care. It is an innovative way to organize
and blend traditional health and social services for frail
or disabled adults. The goals of our Centers are:
- To provide daily
supportive services for members who are physically able
to be at home but require medical supervision,
socialization and nutritional monitoring within a safe,
structured environment during daytime hours.
- To offer a valuable
option of services and companionship for those who find
it difficult to get out by themselves and frequently
feel lonely and isolated.
- To provide a supportive
system to enable the elderly to re-enter community life
from a hospital or nursing home and in making the
transition from independent living to group care.
- To Help mentally or
physically impaired adults to maintain or improve their
level of functioning, for as long as possible.
- To Provide respite to
families and caregivers coping with the twenty-four hour
responsibility of an elder adult or to maintain
employment.
